As the title points out, this applies only for renewing your tourist visa in Medellin. The requirements may be different in other cities. The one thing I can’t figure out is why they arbitrarily give you either 1 or 2 month extensions. The first 2 times I went, I asked for 2 months but was told only 1 was possible, while my friend Stepan asked for 2 months and was given it. On the most recent visit, I again asked for 2 months and my perseverance paid off. Also if anyone knows whether it is 180 consecutive days that you’re allowed to remain in Colombia or if it’s within a calendar year and could provide a link it would be much appreciated. I’m getting conflicting information.
Remember that banks are closed from 11:30 a.m. until 2 p.m. and that the fee can only be paid at a certain bank (req. 1). Also the following is for the first extension. For subsequent extensions, you only need to pay the fee and bring the receipt to the DAS.

1. Consignar $75.500 pesos (COP) en Banco Occidente cuenta corriente No. 263-05464-5 Codigo 103. Traer consignacion original con dos copias.
2. Dos fotocopias de los datos biograficos del pasaporte.
3. Pasaporte original.
4. Dos fotocopias del sello de entrada al pais.
5. Dos fotografias fondo azul 3 x 4.
6. Diligenciar formulario solicitud de tramites con su respective copia.
